Euthanasia: $375
Includes:
- 1 hour appointment allotment
- Sedation, euthanasia and all medications/supplies
- Transport of the body for cremation (cremation fees apply, see “Cremation” information below)
- 2 ink paw and nose prints – Additional ink prints available for $5 ea.
In-home hospice consultation: $200
Includes:
- 1 hour appointment allotment
- Medical record review
- Physical Examination and Environment evaluation – we will review how you can make your pet’s home more hospice-friendly
- Recommendations for therapy options
Cremation
We are able to take your pet for cremation after they have passed. We work directly with Forever Friends for cremation services. Cremation with other companies, burial, or other after-care options must be arranged by the pet owner. For cremation of pets over 100 pounds, we will arrange direct pickup at your home by Forever Friends.
With Forever Friends, all cremations are private. This means that each pet is kept in a separate unit and never mixed with other pets, regardless of whether you choose to have the cremated remains returned to you.
Premium Private Cremation (return of cremated remains)
Service includes:
- Transportation to Forever Friends
- Private cremation
- Clay Paw impression
- Lock of fur on memorial poem
- Choice of either hardwood urn box or biodegradable urn box
- Custom engraving of selected urn or urn box
- Cremation certificate
- Hand delivery of the remains to family’s home
- Presentation in elegant velvet bag

- Transportation to Forever Friends
- Private cremation
- Scattering of cremated remains on private property in a peaceful, secluded area

**TRAVEL FEES**
There is no fee for travel within 15 minute of our location. Travel outside of this will be charged as follows. You can estimate this fee in a maps app by using zip code 43147.

Additional services and fees provided by Comfort Care Mobile:
- Clay Paw Print impressions (provided at the end of euthanasia appointment): $20 each
- Note: These are the paw print only and do not include the pet’s name or any engraving/impressions. These will be soft when given to you and must be baked to harden; instructions will be included with the clay print. Forever Friends is able to provide impressions that include these things and are returned to you baked already.
- Additional ink prints (2 included): $5 each
- Lock of fur in small glass jar: no charge
- After-Hours and Weekend appointments have an after-hours fee of $100
